1. Do you have an Employee Handbook or Human Resources Policies and Procedures Manual?


2. Have your policies and procedures been reviewed for legal compliance within the last twelve months?


3. Do you have policies surrounding key issues such as workplace harassment, disability accommodation, violence in the workplace, and substance abuse?


4. Do you have signed Employee Handbook Acknowledgements for every employee?


5. Do you have the legally required state and Federal posters posted in key locations throughout your facilities?


6. Do you have process forms to track issues such as employee status changes, hours of work, vacations, leaves of absence, overtime, and benefits?

8. Do you maintain medical records and I-9 forms separate from the main personnel file?

12. Have you provided Harassment training and other workplace discrimination training to all employees?


13. Are claims of harassment promptly investigated and corrected?


14. Do you have an I-9 form on file for all employees and do you audit your I-9 forms for compliance at least annually?


15. Do you have a formal disciplinary policy?


16. Have managers and supervisors received training to correctly handle employee performance concerns?


17. Does your employment application include the necessary legal disclaimers and require the applicant to sign?


18. Have your hiring supervisors received interview training advising of the questions they can and cannot ask?


19. If you conduct background checks for new hires, are you in compliance with the Fair Credit Reporting Act?


20. Do you have job descriptions for all positions?


21. Are your positions correctly categorized as exempt or nonexempt?


22. Are performance evaluations conducted at least annually?


23. Do you have a disability accommodation policy and procedure?


24. Are your COBRA or state continuation notice procedures in compliance?


25. Do you know proper record retention rules for personnel files and other HR records?
